See DVM exam. This dog is not placeable due to poor condition it is in. Did not do all vaccines- dewormer or do heartworm test due to not being placeable and being in a comprimised state of health. Treated for fleas with Capstar and Frontline Plus. Administered DA2PP.

Dog is QAR and has a body condition score (BCS) of 3/9 (on a scale from 1 to 9- where 1 indicates severe emaciation- 9 indicates morbid obesity- and a score of 4 or 5 indicates a normal body condition)- but with notable generalized- moderate muscular atrophy. Dog has a pronounced L front limb weight-bearing lameness. Appetite for canned food is excellent. Mouth/Teeth - Severe- very thick dental calculus on almost all teeth; severe gingivitis and gingival recession; R upper 4th premolar appears to be fractured to level of gumline. Unable to examine rostral dentition- as lifting lips or opening mouth wide enough for viewing causes cessation of breath movement for this brachycephalic dog. Ears - Copious moist- dark brown- waxy exudate both ears- with mild-moderate erythema. Hearing sense seems intact. Eyes/Nose - Mature cataracts in both eyes- unable to perform fundic exam due to opacity of lenses. Reduced pupillary light response in both eyes. Copious dried serous discharge at medial canthi and in facial folds. Visual sense seems adequate for movement and light-dark- but is likely impaired generally due to lenticular opacity. Heart/Lung - Notable increased effort/sound during expiratory phase. Increased bronchovesicular sounds all lung fields during both phases of respiration; no crackles or wheezes. No respiratory stertor or stridor noted. Difficult to precisely auscult heart sounds due to ambient increased respiratory noises- but no murmur was heard during auscultation. GI - No obvious abnormality. Reproductive - No obvious abnormality. Did not shave to check for spay scar. Skin /Hair coat - Facial fold dermatitis. ~5mm dermal mass on L lower eyelid. Flea dirt present. Hyperpigmentation of perianal and perivulavar regions- and of medial thighs- inguinal area- and axillae. Toenails have grown so long and curled as to cause toes to splay laterally or medially when paws are placed on floor. L front 5th digit's nail is curled around and applying such pressure into pad as not to allow my fingernail to pass between the nail and pad- but not so much to cause it to enter into pad. Musculoskeletal - Markedly hunched posture. Difficulty with sitting and walking. Crepitus of both stifles and bilateral patellar subluxation. Decreased ROM of R shoulder. Neurologic - No obvious abnormality. Rx Carprofen 25mg PO SID x 7d (1st dose given at exam) Rx Clindamycin 75mg PO BID x 6d
