Depressed- lethargic- weak/mildly ataxic on ambulation. Anorexic today. No vomit or diarrhea noted since 2 days ago- when vomit and formed stool were found in kennel. Tacky- pale pink mm- CRT=1.5sec. T=98.2 degrees F- HR=140- RR=50 with increased abdominal effort. Has lost 4.8 lbs since intake exam 4 days ago. Abdominal palpation - doughy- but supple- no masses palpated. Thoracic ausculation - possible (grade 1-2/6) musical systolic murmur heard PMI R side- but not consistemtly detected. All PLNs palpate WNL. Significant dehydration - estimate 10-12% dehydrated. Parvo test=negative. Administered 800ml LRS SQ. Set in kennel with thick bedding and warm rice bags. Unknown cause for sudden severe apparent anemia- dehydration- depression- etc. Recommend humane euthanasia when SH is up (tomorrow).

Dog was in extremely poor condition this morning. Lethargic- not eating- vomit in kennel- semi-formed very bloody stool and painful when trying to pick up. Dog was euthanzied with OK/consent of prior to DVM arrival. I consulted DVM who had seen the dog the day prior in bad shape and she okayed immediate euthanasia as well. Body was bagged/tagged and placed on shelf for possible necropsy for cruelty/neglect charges from pending case. I have sequenced it to go to the U of M for necropsy but supervisors need to ok this first.

Found dog depressed and laterally recumbent this morning- minimally responsive- painful (tensing and yelping) when attempted to move onto soft bedding. Large amount of soft melena passed in kennel overnight- as well as large volume of vomited fluid on kennel floor- including some coffe-grounds appearance. Character of vomit and feces indicate gross hemorrhage into gastrointestinal tract. Given the condition of this dog- the prognosis is grave regardless of definitive diagnosis for clinical signs and rapid decline in health. Recommend humane euthanasia.

Stray dog- heavily matted and with some superficial skin lesions visible (L dorsal lumbar- R cranial carpus) - lesions are dry- not currently inflamed- and no crusts are noted. No treatment required at this time; however- recommend grooming/clippering matts in order to assess all skin and uncover any possible active/infected lesions.

08/10/17: BAR- severely matted throughout coat Mouth/Teeth - moderate staining upper canines- moderate tartar molars Ears - severe mats on lateral 2/3 of ear pinnae AU; AS- yeast odor- cleaned and applied Tri-otic ear ointment Eyes/Nose - OS slight increased opacity; OD- large increased opacity- possible lens luxation; does not appear to be visual OU Heart/Lung - No obvious abnormality GI - diarrhea; straining to defecate; suspect anal glands need expressiing Reproductive - No obvious abnormality Skin /Hair coat - one 2 cm abrasion abrasion left flank; applied Dermavet ointment to wound; multiple areas of alopecia with pink skin; interdigital dermatitis; coat is severely matted throughout and smells of urine and feces; Mats along rear were impeding the dog's ability to defecate; Shaved around rectum and vulva to alllow defecation and urination Musculoskeletal - No obvious abnormality Neurologic - No obvious abnormality Administered DA2PP vaccine SC and pyrantel pamoate PO; Started on Endosorb: one tablet PO BID x 3 days.
