11/09/18 - lethargic Mouth/Teeth - multiple broken or worn teeth: using Modified Triaden system #104 slab fracture- 204 fractured to level of gingiva- 304 fractured- 404 worn Ears - black debris AU Eyes/Nose - OD slight swelling ventral to eye; moderate discharge OU Heart/Lung - No obvious abnormality; HR = 120 bpm- RR=16 rpm GI - No obvious abnormality- tense on palpation Reproductive - No obvious abnormality. 2 testes in scrotum Skin /Hair coat - generally dirty and dry; bilateral crescent shaped indentations behind the nostral- interdigital dermatitis on all four feet; multiple small (less than 1 cm) pressure sores lateral aspect of both hocks; long nails x 4 Musculoskeletal - significant muscle wasting especially in all 4 limbs; hesitant to stand Neurologic - ataxic in rear- slightly decreased proprioception both rear limbs STT results: OD 10 mm/minute OS 12 mm/minutes HW test results: negative Treatment Plan: trimmed nails x 4; medicated bath- begin refeeding plan; Refeeding Plan: First 2 days: 1/3 cup EN and 1/3 cup puppy TID If no vomiting or diarrhea- increase to 1/4 cup of each per feeding Rechecks: interdigital spaces in all 4 feet- may need chlorhexidine soaks Reweigh in 7 days

11/10/18 - Dog is QAR this am; dark- formed feces- no vomiting; mm slightly pale- crt=2.5 seconds Eye discharge today is bilateral; noted moderate entropion. Entropion along with discharge and low STT results indicative of KCS. Will start on artificial tears. Noted some discomfort when eating dry food-likely due to dental pain from multiple fractured teeth Will add water to dry food and start on short course NSAID to address dental pain; Future care will require significant dental treatment in addtion to reconditioning of musculature as part of rehabilitation plan for this dog Rx: Artificial tears: one strip OU BID x 5 days Carprofen (75 mg/tab) 1/2 tab PO BID x 5 days Advised ACTs to d/c NSAID if GI signs are noted over next 24-48 hours
