11/16/18 - ACT report application of meds to face and eye areas has been variable based on cat's behavior. Brief DVM exams are well tolerated at times and cat offers mild resistance at other times. Appetite is good and cat is quiet with normal postures. Will continue to monitor.

Mild seromucoid nasal discharge and mild congestion. QAR. Advised by ACTs that this cat has become difficult to clean around and they have not been able to apply meds for a few days now due to cat's behavior (hissing and threatening to swat). Will attempt to move to FF ward to continue to monitor URI signs while avoiding contact during kennel cleaning procedure.

Mouth/Teeth - No obvious abnormality- appears to have bit tounge- bruised Ears - Both pinnas are red and warm- possibly frostbitten- especially left ear Eyes/Nose - Left eye has an abrasion above and below and conjunctivitis- abrasion on top of nose Heart/Lung - No obvious abnormality GI - No obvious abnormality Reproductive - No obvious abnormality Skin /Hair coat - No obvious abnormality- dirty- hair around neck looks like he has recently had a collar on. Musculoskeletal - No obvious abnormality Neurologic - No obvious abnormality FeLV/FIV test was negative *Started on medications for in and around left eye. See DVM exam for more info.

11-14-18 - Young intact male white w/black DSH. Appears to have been wearing a collar until recently- based on indentation in haircoat around neck. HR = 200 bpm. Resp WNL- clear. MMs pink. Relatively clean coat. No sign of fleas. Alert & friendly. No microchip found upon scanning. He has dried blood on the sides of both nares- also on chin. Tongue appears to have been bitten on left side. Cannot palpate any fractures on body or face and he shows no reaction or pain response to palpation. Normal withdrawal on all 4 limbs. Has several small abrasions: on bridge of nose- twp to the left of nose below medial canthus O.S.- and 1.5 sq. cm above lateral canthus. This last is deeper than the others. Slight conjunctivitis O.S. Ventral neck and between front legs his coat is a bit dirty and looks as though it had been wet with drooled saliva but dry now. Otherwise NSF. Rx: NeoPolyDex ophth ointment O.S. in and around eye B.I.D. x 3 days.
