12/08/18 - cat presented in sternal recumbency with signigicant head tremors; hx from owner reports sudden anorexia 24 hours ago; neurological signs started today. No reported hx of ingestion of toxins or trauma Mouth/Teeth - teeth normal; mm pale; crt=3 sec- tongue pink- no ulcers- no hypersalivation Ears - No obvious abnormality Eyes/Nose - No obvious abnormality; pupils normal Heart/Lung - No obvious abnormality GI - No obvious abnormality Reproductive - No obvious abnormality Skin /Hair coat - approx tablespoon size white cream type material matted into fur on lateral aspect of R rear limb Musculoskeletal - No obvious abnormality Neurologic - significant head tremors; able to right herself from lateral to sternal- unable to stand from sternal; deep pain present in rear limbs; nonpainful on palpation along topline and rear limbs Primary Rule outs: exposure to toxin- Toxplasmosis infection- FeLV- FIV infection- trauma Tx: bathed right rear limb with Dawn dish detergent to remove cream material administered approx 75 mL LRS SC Place in kennel to monitor for improvement.

12/08/18 - cat's condition has declined in last several hours: mm pale- neurologicl signs increasing to mental dullness. This cat's extremely low body weight combined with current clinical signs are indicative of significant underlying disease process. Because of severity of clinical signs and grave prognosis for recovery- performed humane euthanasia.
